A Practical Guide to the Evaluation and Treatment of Male LUTS in the Primary Care Setting

Rosenberg and colleagues provide the primary care physician (PCP) with a practical guide for the evaluation and treatment of male lower urinary tract symptoms (LUTS). The authors argue that PCPs can safely treat men for LUTS in their office, allowing, theoretically, more men with LUTS to be treated effectively and efficiently, reserving refractory cases for the specialists. Thorough definitions of LUTS, benign prostatic hyperplasia (BPH) and overactive bladder (OAB) are provided with the pathophysiology of each. The authors present an algorithm for the PCP that gives a logical framework for deciding when to initiate therapy and when to refer. The article also provides how to identify and evaluate the presence of LUTS, how to assess bother, how to differentiate between OAB and BPH and how to use post-void residual measurements to screen the patient. This article can be shared with your referring, primary care providers and presents an opportunity for a collaborative effort to more fully address the needs of the aging male population.
