Private Payer Reimbursement for The Spanner™

The insertion of The Spanner has been covered by a number of private insurance companies.  Private payers that have either approved payment or covered the insertion of The Spanner include, but are not limited to the following: 

When submitting claims for CPT code 0084T to private payers:

• Prior Authorization/Pre-Determination with Carrier
• Secure a signed Waiver of Liability form (PDF) from the patient
• Submit claim using CPT Code 0084T and any appropriate modifiers
• Appeal denied claims
